ALL shipments must include return documentation. Undocumented shipments may be refused.
ALL shipments MUST be properly decontaminated PRIOR to shipment to our facility. In addition, all confidential and patient data shall be removed from any and all databases on the equipment, as per HIPPA guidelines and regulations, prior to shipment to our facility.
Equipment Placement Services cannot accept bio-hazard material or medical waste of any sort or kind at any time. All waste and/or bio-hazards shall be properly and legally disposed of PRIOR to shipment of the asset to our facility.
